WARNING - READ CAREFULLY

IN CONSIDERATION of being allowed to participate in the Manitoba Amateur Wrestling Association (MAWA) and related events and activities, the undersigned:

1. AGREES that prior to participating, I will inspect the facilities and equipment to be used, and if I believe anything is unsafe, I will immediately advise the Supervisor/Trainer of such related events and activities and refuse to participate.

2. ACKNOWLEDGES AND FULLY UNDERSTANDS that I will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from my own actions, inactions or negligence, but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks unknown or not reasonably foreseeable at this time.

3. ASSUMES all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability, or death.

4. RELEASES, WAIVES , DISCHARGES AND COVENANTS not to sue the MAWA, its agents, coaches, or other participants, and if applicable, owners and lessors of the premises used to conduct the practices or events, all of which are hereinafter referred to as the Releasees, from any and all liability to each of the undersigned, his or her heirs and next of kin, for any and all claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise.

5. COVENANTS AND AGREES not to make any claim or to take any proceedings on account of any serious injury, including permanent disability or death, and severe social and economic losses against any person or persons, form or corporation who may claim contribution or indemnity from the Releasees, under the provisions of any statute or otherwise.

6. RELEASES, DISCHARGES, HOLD HARMLESS AND INDEMNIFIES the Releasees, from any and all liability for the damages to any third party resulting from my participation in any event or activity related to the MAWA.

7. CONFIRMS that I am physically capable and fit to participate in this activity.

I HAVE COMPLETELY READ AND UNDERSTAND THIS WAIVER AND RELEASE AND ITS TERMS. PRIOR TO SIGNING THIS WAIVER AND RELEASE OF LIABILITY I HAVE HAD THE OPPORTUNITY TO ASK ANY QUESTIONS ABOUT THIS WAIVER AND RELEASE OF LIABILITY. I HAVE READ AND SIGNED THE ABOVE WAIVER AND RELEASE, AND I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
